Fees & Insurance

Clear, upfront information about cost, out-of-network benefits, and how to use your HSA/FSA.

Where we stand with insurance

This is an out-of-network practice with all insurance plans. That's a deliberate choice, not a limitation: it lets each appointment be a full, unrushed, one-on-one session with a vestibular specialist — without an insurer dictating how long we meet or how many visits you're "allowed."

Payment is due at the time of service. We accept all major credit cards, plus Health Savings Account (HSA) and Flexible Spending Account (FSA) cards.

What's a superbill?

A superbill is an itemized receipt for your visit. It includes the diagnosis and treatment codes your insurance company needs to process an out-of-network claim. Even when a practice is out-of-network, many plans still reimburse a portion of the cost — and a superbill is the document that makes that possible.

How reimbursement works

Here's what to expect:

  • You pay at the time of service.

  • An itemized bill is emailed to you or submission to your insurance.

  • Any reimbursement is sent directly to you by your insurance carrier.

  • That reimbursement may count toward your individual or family out-of-network deductible, depending on your specific plan.

Because every plan is different, the amount and timing of reimbursement vary. We're glad to help you understand the process before you commit.

Using your HSA or FSA

Vestibular physical therapy is an eligible medical expense, so you can pay with your Health Savings Account (HSA) or Flexible Spending Account (FSA) card — using pre-tax dollars you've already set aside for care. It's one of the simplest ways to make specialized treatment more affordable.

Common questions about cost

Is cash-based physical therapy actually more expensive?

Often less than people expect — and sometimes less overall. It's worth comparing the total cost of care, not just the per-visit sticker price. With in-network PT, you may still owe copays or be paying down a deductible at every visit, and insurer-imposed visit limits or short sessions can stretch treatment over many more appointments. Specialized, one-on-one vestibular care is designed to get to the right diagnosis and plan efficiently — and some conditions, like BPPV, often resolve in just one or two visits. Fewer, more focused sessions can mean less time and less total spend than a longer in-network course of care.

Will my insurance reimburse me?

Many out-of-network plans reimburse a portion of the cost, but coverage varies widely. The best step is to check your specific out-of-network benefits — we can help you understand what to ask.

Can I use my HSA or FSA?

Yes. Both are accepted.

Do I need a referral?

No. You can book directly. If your evaluation suggests you'd benefit from additional medical workup, we'll coordinate with your physician.

Do you accept Medicare?

No. The Balance Lab is not a Medicare-approved provider, so physical therapy services cannot be billed to or reimbursed by Medicare.
